STEP 1 - Complete EVITP Recertification LOOK UP Form
After entering your certificate number, first & last name, Click on “LOOK UP” to open the EVITP Recertification Verification Form.

STEP 2 - EVITP Recertification Verification form:

• If the infromation on the EVITP Recertification Verification form is correct, click on "Request Payment Code"
  Payment code and payment link will be emailed to you.
  
  Make payment to receive credentials to access EVITP Recertification course (via email)


• If information on the EVITP Recertification Verification form is incorrect, click on "Verify Form". The EVITP Recertification Submission Form will open.

Step 3 - EVITP Recertification Submission Form
Note: (step 3 is only required if we do not have your current or complete information on file)

• Update form with your correct information
• Upload photo ID
• Agree to Privacy Policy
• Submit request payment code and payment link
  Payment code and payment link will be emailed to you.
  
  Make payment to receive credentials to access EVITP Recertification course (via email)
